upload all required documents above.

My signature also certifies that should I provide AdultCamStaffing with false identification (including name and/or date of birth), I will not hold AdultCamStaffing liable as I understand that this identification was accepted in good faith, and that this effort was made to ensure that I am at least 18 years of age.
I also certify that all decisions related to the content of my Studio performances are made at my own discretion, and that my Studio performances do not violate the local community standard of "obscenity" in my area, and that my Studio performances do not violate any other laws. I am completely responsible for my own actions. I indemnify and hold harmless AdultCamStaffing from all related liabilities. I hereby discharge all persons related to AdultCamStaffing of any past, present or future liability.
I hereby represent that I am at least 18 years of age and that my age is equal to or higher than local, community, or state laws in my area require for mature online content. I further represent that I have fully read this document prior to its execution. I have not been induced to sign the same other than by recited considerations, by any statement made by AdultCamStaffing, its agents, employees, or anyone acting on their behalf. 1. StudioContractor understands that by transmitting their performers video, permitting their pictures to be displayed on any website on the world wide web owned/operated by AdultCamStaffing ,( including any mobile devices ,TV, DVDs, printed media )that any persons who surf the web can see that performer at any time. It is also understood that AdultCamStaffing cannot be responsible for people outside of the Company to copy and reproduce any or all video and pictures coming from performers show. It is clearly understood that by doing live video on the Internet can at some times create a moral problem for the hiring of performer in an occupation other than doing the live video. The StudioContractor agrees in no way is AdultCamStaffing responsible for the termination or failure to get a job that is clearly in fault of the live video shows that artist has done for the network.
2. StudioContractor agrees that if anyone else which will appear in the transmission of performer video they will submit all of their own paperwork to appear on the Network. If any other person nude or non nude are visible the contractors account will be blocked.
3. StudioContractor agrees to use performers real pictures and video for the purpose of being a performer for AdultCamStaffing . Contractor also assigns the Company, its successors , and those acting under its permission or upon its authority, permission to copyright and/or publish any photograph without using contractor's name, and to keep such photographs, portraits, in such manner as the Company shall deem proper to their use. I also understand that by signing this agreement that i lose all copyrights to all images and videos publlicated or streamed through AdultCamStaffing.
4. StudioContractor hereby gives the Company permission for advertising purposes to permanently publish photographs on Cammatrix.com websites and any other websites owned/operated by AdultCamStaffing including but not limited to the previously mentioned website.
5. Company does not seek and does not assert any control over the activities of StudioContractor in the performance of services necessary to accomplish the objective sought by any client. StudioContractor understands and agrees that is an independent contractor who, as such, is solely responsible for exercising independent judgment as to the time, place, and manner in which the Services are performed.
6. StudioContractor understands that they are an Independent Contractor, therefore there is no Workmen’s Compensation provided by Company for injuries that occur during transmission of your show for our network.
7. StudioContractor understands that no medical benefits or any other benefits thereof will be provided by Company.
8. No taxes will be taken out of StudioContractor’s paycheck therefore taxes on money paid to Contractor must be declared by the Contractor to their IRS. ( government )
9. It is understood that any excessive charge backs incurred by the Company due to fraudulent activity for shows done on the Network by the performers of the StudioContractor will be debited from the StudioContractorr’s account.
10. Company shall hold money from StudioContractor’s check or wire for the amount of the charge backs and any costs involved with such charge back, if such a chargeback/refund occurs.
11. StudioContractor agrees not to divulge any information pertaining to business or how business is conducted to anyone in reference to the Company.
12. Company shall immediately release any contractor for redirecting or otherwise misdirecting clients so a loss of income for the Company is achieved . Therefore forfeiting any money due to said Contractor.

This Agreement sets forth the full and complete understanding between StudioContracter and AdultCamStaffing with respect to its subject matter, and supersedes all prior understanding or agreements, whether written or verbal. This Agreement may be modified upon notice by AdultCamStaffing to its Contractors. Unless contrary to law or otherwise stated, each provision of this Agreement shall survive termination. This Agreement shall be governed by, and construed in accordance with, the laws of Netherlands for contracts entered into and wholly performed within that Country.

Please be advised that all personal data supplied to AdultCamStaffing will be

kept confidential, unless it is necessary to release the information if

called upon by law-enforcement agencies investigating a crime (i.e., child

pornography, cruelty to animals, etc.)
